Diabetic Educator

Description:Overall responsibility for community health promotion/disease prevention programs including coordination of State of Maine Community Health Project. Will coordinate the Diabetes Prevention and Control Program. Menu planning for the elders, diet counseling for diabetes and other nutritional diseases.

**** Must speak Spanish Fluently.

Duties:1.Provide nutritional counseling to clients referred by the Medical Director, mid-level practitioners, or other physicians
2. Prepare a monthly menu for the Senior Meal program
3. Work with the cook and senior citizens in the menu preparation
4.Preparation of annual goals and objectives for nutrition program
5.Coordinate and implement Diabetes Control and Prevention
6.Work with the Community Health Nurse to coordinate health education activities for diabetes education and prevention
7.Coordinate all community health education which will include, but not be limited to: cancer, heart disease, hypertension, diabetes, tobacco abuse, and school health education
8.Prepare modified diets for clients with specific health problems
9.Provide instruction to clients in the use of modified diets
10.Attend outside meetings as requested
11.Attend continuing education workshops as requested or as required to maintain certification
12.Provide in-service training to other staff
13.Maintain a library of accurate and appropriate education materials as an adjunct to counseling and education
14.Development of a referral system of community resources for nutrition and food assistance
15.Provide an initial nutritional assessment for each client referred by the counseling program. Periodic assessments will be completed as directed
16.Be responsible for following confidentiality procedures as outlined in the Privacy Act
17.Other duties as assigned by supervisor
Qualifications:BS/BSc. State of Texas licensed Registered Dietitian, eligible to take the examination to be a Certified Diabetes Educator within six (6) months of employment, 2-4 years of experience in menu planning, diet counseling, community education and coordination, ability to work with the public, writing skills and exercise background. Fluent in Spanish.
